The acceptability and feasibility of group cognitive behavioural therapy for older adults with generalised anxiety disorder

Background:
Group psychotherapy for older adults with generalised anxiety disorder is an under-researched area.
Aim:This report describes a mixed method evaluation of the acceptability and feasibility of an Overcoming Worry Group.
Method:The Overcoming Worry Group was a novel adaptation of a cognitive behavioural therapy protocol targeting intolerance-of-uncertainty for generalised anxiety disorder, tailored for delivery to older adults in a group setting (n = 13).
Results:The adapted protocol was found to be acceptable and feasible, and treatment outcomes observed were encouraging.
Conclusions:This proof-of-concept study provides evidence for an Overcoming Worry Group as an acceptable and feasible group treatment for older adults with generalised anxiety disorder.
The cognitive developmental profile associated with fragile X syndrome: A longitudinal investigation of cognitive strengths and weaknesses through childhood and adolescence

Few studies have investigated developmental strengths and weaknesses within the cognitive profile of children and adolescents with fragile X syndrome (FXS), a single-gene cause of inherited intellectual impairment. With a prospective longitudinal design and using normalized raw scores (Z scores) to circumvent floor effects, we measured cognitive functioning of 184 children and adolescents with FXS (ages 6 to 16) using the Wechsler Scale of Intelligence for Children on one to three occasions for each participant. Participants with FXS received lower raw scores relative to the Wechsler Scale of Intelligence for Children normative sample across the developmental period. Verbal comprehension, perceptual organization, and processing speed Z scores were marked by a widening gap from the normative sample, while freedom from distractibility Z scores showed a narrowing gap. Key findings include a relative strength for verbal skills in comparison with visuospatial–constructive skills arising in adolescence and a discrepancy between working memory (weakness) and processing speed (strength) in childhood that diminishes in adolescence. Results suggest that the cognitive profile associated with FXS develops dynamically from childhood to adolescence. Findings are discussed within the context of aberrant brain morphology in childhood and maturation in adolescence. We argue that assessing disorder-specific cognitive developmental profiles will benefit future disorder-specific treatment research.

The UK incidence of prostate cancer has been increasing in men aged < 60 years. Migrant studies and global and secular variation in incidence suggest that modifiable factors, including a high-fat diet, may contribute to prostate cancer risk. The aim of the present study was to investigate the role of dietary fat intake and its derivatives on early-onset prostate cancer risk. During 1999–2004, a population-based case–control study with 512 cases and 838 controls was conducted. Cases were diagnosed with prostate cancer when ≤ 60 years. Controls were sourced from UK GP practice registers. A self-administered FFQ collected data on typical past diet. A nutritional database was used to calculate daily fat intake. A positive, statistically significant risk estimate for the highest v. lowest quintile of intake of total fat, SFA, MUFA and PUFA was observed when adjusted for confounding variables: OR 2·53 (95 % CI 1·72, 3·74), OR 2·49 (95 % CI 1·69, 3·66), OR 2·69 (95 % CI 1·82, 3·96) and OR 2·34 (95 % CI 1·59, 3·46), respectively, with all P for trend < 0·001. In conclusion, there was a positive statistically significant association between prostate cancer risk and energy-adjusted intake of total fat and fat subtypes. These results potentially identify a modifiable risk factor for early-onset prostate cancer.
Adherence to Principles of Motivational Interviewing and Client Within-Session Behavior

The purpose of this study was to examine whether counselor adherence to Motivational Interviewing (MI) principles was associated with more productive within-session client behavior in a smoking cessation trial for African American smokers. For these analyses 89 baseline counseling sessions of the trial were audiotaped and coded using the Motivational Interviewing Skill Code (MISC). Counselor adherence indicators included a global subjective rating of MI adherence and the frequency of MI-consistent and MI-inconsistent counselor behaviors described in the MISC. Indicators of productive client behaviors included global subjective ratings of within-session client functioning and counselor-client interaction, as well as the frequency of statements by the client favorable toward changing behavior (“change talk”) and resistant regarding changing behavior (“resist-change talk”). Results provided support for the principles of MI. Counselor adherence indexed by both the global subjective rating and MI-consistent behavior frequency was significantly positively associated with global ratings of within-session client functioning and counselor-client interaction, as well as more change talk.

Summary
Introduction
For its sheer scale, the wide array of service activities in which it possesses competitive advantage by both national and international standards, and for its cosmopolitanism and international connections, London clearly stands out among British cities. It is also, in simple terms that can too easily be lost sight of in an era of self-conscious globalisation, the national capital of the UK: economically, socially and culturally, as well as politically. As such, it plays a crucial role in the British urban system, with important implications, both complementary and competitive, for the way in which other city-regions can function and develop. Understanding how its role and performance are evolving in the new kind of intense internationally competitive economic environment that has been emerging over the past 20 years or so, has an importance that goes well beyond both the rather privileged areas of south east England, or what it has to show about the development of ‘global cities’.
The things which make London most distinct in a British context, however, namely its sheer scale and diversity, both physically and in population terms, also mean that an unusually wide cross-section of urban situations, environments and issues can be found (and observed) somewhere within its functional region. Often this is on a scale which makes them more visible, statistically and politically, than in smaller places. There are obvious exceptions in that London had little of the kind of heavy industries, whose demise has been so traumatic in many northern cities, and has benefited from the general strength of the southern regions since the 1920s. Comparison with Glasgow, Liverpool and Manchester in particular among cities studied in this programme highlights ways in which these basic regional economic and structural facts matter. But equally, a London study can display rather clearly a number of processes which are underway in some parts of (or affecting some groups in) all city-regions, producing similar kinds of outcome and giving rise to what may be very similar policy issues.
The research reported here, like that of the Bristol, Liverpool–Manchester and Edinburgh–Glasgow teams (see Chapters Two to Four of this volume), was designed to be ‘integrative’, rather than to be focused on a specific theme or activity (as projects reported in the remaining chapters of this volume are).
